DECLARATION

I declare that to the best of my knowledge and belief:

1. All information provided above is true
2. All material particulars affecting the assessment of the risk have been disclosed
3. The car is in a sound and roadworthy condition; I clearly understand that any "Pre-Existing Damages" will not be covered in my Insurance / Takaful policy and pre-existing damages, if any, will be communicated to the Company.
4. I confirm the existence of "Insurable Interest", where vehicle "Registration Ownership" and "Insured Person / Participant" are the same person; failing which any claim under the policy will not be entertained.

I agree that this proposal and declaration shall be the basis of the contract between me and the company and shall be deemed to be the incorporated in such contract. I undertake that the car or cars to be insured / covered shall not be driven by any person who to the best of my knowledge has been refused any motor vehicle insurance / takaful or continuance thereof.
